Admission and Immunization Requirements
Students entering a Tennessee school for the first time must present a certified certificate of birth, a social security
card, and Tennessee Certificate of Immunization and Physical Examination form. Students entering kindergarten
must have reached the age of 5 on or before September 30th of the current school year. Children entering school (K-
12) must have the required immunizations listed below or the appropriately documented exemption. Students
without proper documentation for immunizations will be sent home until documentation is provided. (Board Policy

For Kindergarten entry (3 doses) and 7th grade (For adolescents, vaccines with either a 2 or 3 dose schedule are available. |
MMR |

Given after the 1st birthday. Second dose given at least 30 days after the 1st dose. |
VARICELLA (Chicken Pox) |

Parental or Physician history of disease is acceptable. |
NOTE: Hepatitis B immunizations are required before a student enters the 7th grade. If your child already meets these qualifications,
please present the documentation upon registration (unless documentation has already been placed in your child's permanent record). If your
child does not meet the requirement, contact your doctor or the local Health Department. Students will not be allowed to register without
documentation
